Monthly Check-in Form
Each month, we’ll send you a brief check-in form called the “Sunlight GLP-1 Global Monthly Check-in Form.” It takes about 2 minutes to complete and is a chance to share how you’re feeling, how the medication is working, and anything you’d like your provider to know.
Your provider uses your responses to personalize your treatment, which can include reviewing your current dose. The more your provider knows, the better they can tailor your plan to you.
No action is needed if nothing has changed. Your prescription will continue as scheduled

Sunlight will email and text you each month with a link to your check-in form.
Questions or side effects?
If you have questions or would like to discuss side effects with a provider, message your care team through the Sunlight patient portal under ‘Communications’ in the left side menu.
